56 of Texas’s 1177 Medicare-certified nursing homes carry the CMS High Performing Facility icon on Nursing Home Care Compare as of CMS’s October 2026 list. That’s 4.8% of the state, below the national share of 11.6%.
Here’s the full list by county, what the icon records, and what to do if you run one of these buildings or compete with one.
What the icon means
CMS places a trophy on a facility’s Care Compare profile when it qualifies for the Risk-Based Survey, a shorter inspection reserved for buildings with a clean record. Qualifying takes 11 checks at once: a 5-star overall rating, at least 3 stars for staffing, no harm-level citations in the last cycle, an inspection within 18 months, clean staffing and assessment data audits, no staffing waivers, no recent change of ownership, and more.
The list rebuilds every quarter, and the icon stays “until the facility is no longer eligible.” The full explainer covers what that means in practice.

The largest groups sit in Dallas (4), Harris (4), Bexar (3). 41 of Texas’s counties have at least one qualifying building.
If your building is on the list
You have roughly a quarter before the next rebuild to turn the icon into inquiries, tours, and referral conversations. The 7-surface rollout covers the website, Google Business Profile, per-facility social, the lobby, admissions, hospital partners, and current families, with one dated sentence that stays accurate everywhere. The hospital referral piece is the surface with the highest return.
Write the removal plan before the first post. The icon is a status, and statuses end.
MOZART&CO. runs this as a coordinated recognition campaign for every qualifying building in a network, in one week, tracked in the Marketing Hub so the next quarter triggers an update instead of a scramble. We work with operators across Texas.
If it isn’t
About 88.4% of nursing homes nationally are in the same position, and many of them are strong buildings that missed on a technicality: the survey clock, an acquisition, a staffing star. Find out which of the 11 checks you missed. Three of them respond to work you control this quarter.
About this list
Source: the High Performing Icon column of the CMS Provider Information dataset (NH_ProviderInfo_Sep2026_3.csv) on the Provider Data Catalog, retrieved 2026-10-19. Facility names, cities, counties, and bed counts from the CMS Provider Information dataset as of 2026-10-19. CMS notes that the lists sent to state survey agencies and the icons shown on Care Compare will not always match because of data transmission and posting delays; for any single facility, the Care Compare profile is the reference. This page updates when CMS rebuilds the list.

Frequently asked questions
- How many Texas nursing homes have the CMS High Performing Facility icon?
- 56 of 1177 Medicare-certified nursing homes in Texas, or 4.8%, as of CMS's October 2026 list. The national share is 11.6%. CMS rebuilds the list every quarter, and this page updates when it does.
- What does the trophy icon mean on Care Compare?
- The facility qualified for CMS's Risk-Based Survey by clearing 11 checks last quarter, including a 5-star overall rating, at least 3 stars for staffing, no actual harm or immediate jeopardy citations in the last survey cycle, an inspection within 18 months, and no recent change of ownership. It stays on the profile page until the facility is no longer eligible.
- Why is a 5-star Texas nursing home missing from the list?
- The 5-star overall rating is 1 of 11 checks. Common misses are the 18-month survey clock, which depends on the state's inspection backlog, a change in ownership since the last standard survey, a staffing rating under 3 stars, or an inspection score above the Texas median. Some strong buildings simply wait on the next survey.
- How often does the Texas list change?
- Quarterly. CMS compiles the qualifying list at the end of each quarter and posts it to the Provider Data Catalog and Care Compare, and the icon comes off a profile as soon as a facility stops qualifying. CMS also notes the lists it sends to states and the icons on Care Compare will not always match on timing.
