The trophy icon on Nursing Home Care Compare: what it means for families, and what it doesn't

Starting September 30, 2026, some nursing homes on Medicare.gov show a gold trophy. It marks facilities that qualified for CMS's Risk-Based Survey. What that takes, why it can disappear, and the 5 questions to still ask on a tour.

The trophy icon on Nursing Home Care Compare: what it means for families, and what it doesn't

You’re comparing nursing homes on Medicare.gov and one of them has a small gold trophy next to its name. The others don’t. It’s new, it went up on September 30, 2026, and nobody at the facility has explained it yet.

Here’s what it records, what it leaves out, and what to still ask when you visit.

What the trophy records

CMS calls it the High Performing Facility Icon. A nursing home gets one when it qualifies for something called the Risk-Based Survey, a shorter version of the annual state inspection that CMS reserves for facilities with a clean record. CMS estimates about 12% of nursing homes qualify.

To qualify, a facility has to pass 11 checks at once. The ones that matter most to a family:

  • A 5-star overall rating on Care Compare, and at least 3 stars for staffing.
  • No citations for actual harm, immediate jeopardy, or substandard quality of care in the last survey cycle.
  • An inspection score at or better than the median for the state.
  • An inspection within the last 18 months, and no failed audits of its staffing or resident-assessment data.
  • No change in ownership since the last inspection.

So the trophy tells you: recently inspected, no serious findings, staffed at a reasonable level, data that held up to audit, stable ownership. That’s a lot of good news in one symbol.

What it leaves out

Read it as a checklist result. Eleven checks, passed as of the last quarter.

It says nothing about the dementia program, the food, how the night shift treats your mother, or whether anyone will call you back. Those are things you learn in the building.

It’s also a snapshot. CMS rebuilds the list every quarter, and the icon stays “until the facility is no longer eligible.” A serious complaint finding or a change of owner can remove it. If a trophy you saw last month is gone, that’s worth a direct question. It may also be a data delay: CMS itself says the lists it sends to states and the icons on Care Compare won’t always match.

And the trophy’s absence is weak evidence. Roughly 88% of nursing homes won’t have one. Some are struggling. Others missed on a technicality: a facility that changed hands last year can’t qualify until its next full inspection, and neither can one the state hasn’t gotten to in 18 months, which is common given inspection backlogs. A 5-star building without a trophy may simply be waiting on the state.

Some resident advocates have raised a separate concern: a shorter inspection means less oversight of the buildings that get it. That’s a policy debate, and worth knowing as you decide how much weight to put on the icon.

What to ask on the tour

Whether or not the trophy is there, these 5 questions get you further than the icon does.

  1. What was in your last inspection report, and what changed because of it? Every facility has findings. The good ones can tell you what they did about them.
  2. What’s your staffing on nights and weekends, in real numbers? Star ratings average things out. You want the shift your parent will actually live on.
  3. How will you tell me when something goes wrong? Ask for the specific process, and who calls.
  4. Who’s the administrator, and how long have they been here? Stable leadership is behind almost every clean record.
  5. If you have the trophy, what would make you lose it, and how would you tell families? A facility with a real answer is a facility paying attention.

If you run the building

Operators reading this: the questions above are the interpretation you’re competing with. Families won’t research Appendix A. They’ll see a trophy, form a feeling, and test that feeling against the person who answers the phone.
